Making sure your child is immunized helps to keep him healthy, as well as other children attending his school. A list of state-required immunizations from the Ohio Department of Health is available here.
Additional Immunization Requirements for Incoming Seventh and Twelfth Graders:
Seventh Graders: The State of Ohio requires a booster dose of the Tdap (tetanus, diphtheria, and acellular pertussis) and the MCV4 (Meningococcal) vaccine. The Meningococcal vaccine is a new requirement for entry into the seventh-grade.
Twelfth Graders: The State of Ohio requires a second dose of the MCV4 (Meningococcal) vaccine for entry into the twelfth grade.
Once your child has received the required vaccinations, please provide a copy of the shot record to your school’s nurse. Feel free to contact the school nurse with any questions or concerns.

Franlin Co. Public Health offers childhood and adult immunization clinics year-round. Appointments are required and you MUST provide the following at your appointment:
- Vaccination record
- Parent/guardia photo ID
- Insurance card
