Groveport Madison Schools has partnered with FinalForms, an online forms and data management service that is replacing nearly all paper forms needed by our schools and the school district.

FinalForms allows parents to complete and sign enrollment, back-to-school, field trips, and athletic participation forms for your child. The most exciting news is that FinalForms saves the information you submitted from season-to-season and year-to-year, meaning that you should almost never need to enter the same information twice! FinalForms also pre-populates information wherever possible for each of your students, saving you valued time.

Creating a FinalForms Account

To create a FinalForms Account:

   1.  Go to: https://groveport-oh.finalforms.com/ 
   2.  Locate the parent icon and click NEW ACCOUNT below.
   3.  Type your YOUR NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER.
   4.  Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. 


NOTE: You will receive an email prompting you to confirm and complete your registration within two minutes. If you do not receive an email, then check your spam folder. If you still cannot locate the FinalForms email, email [email protected] informing our team of the issue. 


   5.  Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
   6.  Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
   7.  Click REGISTER STUDENT for your first child.

To sign in to an existing FinalForms Account, simply: 

   1.  Go to: https://groveport-oh.finalforms.com/
   2.  Select the appropriate icon for "Parent," "Student," or "Staff" 
   3.  Type in the email address used when creating your account.
   4.  Type in the password used when creating your account.

Resetting Your Login Email or Password

Resetting Password
If the student or parent has forgotten their email address or password to sign into their FinalForms account, just click on the "Forgot your email?" or "Forgot your password?" hyperlink button. Instructions on resetting your account will be emailed to either the student or the parent (depending on which account you're trying to access). The District cannot reset the email address or password for student or parent FinalForms accounts; it can only be done by the account owner.

Student Sign-In Instructions

Students sign into FinalForms as follows:
Username - Last Name plus six-digit Student ID [email protected]                                       (example: [email protected])
 
Password - Student's eight-digit date of birth (example if the date of birth is Feb 14, 2001, you would use 02142001)
