Ohio Law states that all employed minors between 14 and 18 years of age must have a work permit (age and schooling certificate). A new work permit is required each time the place of employment changes.
Please download the Work Permit Application, read the instructions and complete all the information before returning it to Groveport Madison High School, 4475 S. Hamilton Rd., Groveport, OH 43125. UPDATE: During the governor's "Stay at Home Order", please email Becky Fasone to obtain a work permit.
