TO BE ELIGIBLE TO TRY-OUT
- Physical – You must have a current “MEDICAL ELIGIBILITY FORM” (part of the OHSAA Pre-participation Physical Evaluation) uploaded on FinalForms to participate in any football related activity – camp, practice, scrimmage or game. A physical is good for one calendar year. Well visit paperwork DOES NOT count as a physical. The athlete must have an OHSSA physical form signed by a doctor.
