Report Your Positive COVID-19 Test Result

This online reporting form is to be completed by parents/guardians of Groveport Madison students, or Groveport Madison staff members who have tested positive or have symptoms of COVID-19. It is critically important that parents still notify the school's office that their child is absent for the day and for staff to report their absence in the AESOP/Frontline system.
 
Below is updated guidance to clarify the COVID-19 guidelines sent out recently. Public health policies have evolved throughout the pandemic, and the current guidance may be updated again in the coming weeks as we learn more about effective approaches to control the spread of the omicron variant. Any new guidance from Franklin County Public Health regarding COVID-19 policies will be communicated as quickly as possible.
 
Masking Requirements and Recommendations
Masking is still required for everyone in all Groveport Madison School District buildings and events. Regardless of vaccination status, this requirement must be followed by all students, faculty, staff, and visitors. Properly fitted masks are critical to reducing the transmission of COVID-19 within classrooms and during extracurricular activities.
 
COVID-19 Scenarios Based on Vaccine or Booster Status
Under Ohio law, your vaccination or booster status does not impact any of the following scenarios.
 
Student or Staff Members Testing Positive for COVID-19
Students or staff members who have received a positive COVID-19 diagnosis must isolate for a minimum of five (5) days. If the infected person has recovered and is no longer showing symptoms of COVID-19, they may return to school or work after the five-day isolation period. The date of the start of symptoms is Day 0. If the student/staff member is asymptomatic, the test date is Day 0. For example, if the asymptomatic student/staff member tests positive on a Wednesday (Day 0), they may return to school on Tuesday (Day 6), assuming they continue to experience no symptoms.
 
Student or Staff Member is Identified as Being a "Close Contact" to a Person Infected with COVID-19
If asymptomatic, the student or staff member may attend school/work while wearing a mask at all times. Taking a COVID-19 test five (5) days after the last exposure to the COVID-19 case is recommended. If symptomatic, the student or staff member should be tested for COVID. If the student or staff member tests positive, refer to the information above. If the test result is negative for COVID, they may return to school/work while wearing a mask, if they feel well enough to do so.
 
Student or Staff Member is Displaying COVID-19 Symptoms but Has No Known Contact
First and foremost, isolate at home. Identify the date symptoms began and complete our online reporting form. The student or staff member should be tested for COVID-19 as soon as possible. If the test results are positive, refer to the information above. If the test results are negative, they may attend school while wearing a mask at all times, if they feel well enough to do so.  
 
COVID-19 Symptoms Include:
  • Fever (100.4 F) or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea
