Creating a FinalForms Parent Account

At Groveport Madison Schools, we are committed to making the enrollment process as smooth as possible for families. Whether your child is new to our district or transferring from another program, completing enrollment through FinalForms is the first step.

Please review the information below to create your FinalForms parent account and submit all required documentation. Enrollment is considered complete only after all forms have been submitted and approved.
 

Creating a FinalForms Parent Account (for special ed)

Follow these steps to set up your account:

  1. Go to https://groveport-oh.finalforms.com/

  2. Locate the Parent icon and click New Account.

  3. Enter your first name, last name, date of birth, and email address.

  4. Select “I’m not a robot” and click Register.

  5. Check your email for an Account Confirmation Email from FinalForms.

    • Follow the instructions in the email to confirm your account and create a password.

    • If you do not receive the email within two minutes, check your spam folder.

  6. Once logged in, click Add Student in the top-right corner to begin completing forms.

  7. Under Uncommon Situations, select External and enter the name of the education program your student is currently attending.

  8. Complete all required sections highlighted in red. When the sections turn green, your forms are finished.

    • Note: Your student’s application is not complete until all required documents have been uploaded and approved.

 

Parent Forms to Complete in FinalForms (External)

When enrolling a student for special education services, please complete only the forms listed below:

  • New Student Registration Form – Upload your child’s birth certificate and proof of address

  • Contact Information – Parent/guardian phone number and email address

  • Demographic Information – Student’s legal information, date of birth, and address

  • Additional Student Information – Health and emergency details; custody notes if applicable

  • Records Release – If required by Groveport Madison (confirm with Student Services)

  • Medicaid Consent – Permission for Groveport Madison to bill Medicaid for eligible services

Important: Do not complete building-based forms such as transportation or athletics unless instructed by the district.

 

Required Documents for Enrollment

Be prepared to upload the following documents when completing your student’s registration:

  • Child’s birth certificate

  • Parent/guardian photo ID

  • Proof of address (for example, a utility bill or lease agreement)
