Groveport Madison Schools is excited to introduce FinalForms, our new online platform for completing school forms. Whether you're enrolling your child, signing field trip permission slips, or submitting athletic paperwork, FinalForms makes the process faster, easier, and more convenient.

The best part? Once you enter your information, FinalForms saves it—so you won’t have to retype the same details year after year. It even fills in known information for each of your students to help you save time.

creating a finalforms account

To create a FinalForms Account:

   1.  Go to: https://groveport-oh.finalforms.com/ 
   2.  Locate the parent icon and click NEW ACCOUNT below.
   3.  Type your YOUR NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER.
   4.  Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. 

NOTE: You will receive an email prompting you to confirm and complete your registration within two minutes. If you do not receive an email, then check your spam folder. If you still cannot locate the FinalForms email, email support@finalforms.com informing our team of the issue. 

   5.  Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
   6.  Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
   7.  Click REGISTER STUDENT for your first child.

To sign in to an existing FinalForms Account, simply: 

   1.  Go to: https://groveport-oh.finalforms.com/
   2.  Select the appropriate icon for "Parent," "Student," or "Staff" 
   3.  Type in the email address used when creating your account.
   4.  Type in the password used when creating your account.

resetting your login email or password

Need help logging into FinalForms?

No worries! If you or your student forgets your FinalForms email or password, just click the "Forgot your email?" or "Forgot your password?" link on the login page. You'll get step-by-step instructions sent directly to your email to help you reset your account.

Please note: For security reasons, only the account owner can reset login information—our district staff isn’t able to make those changes for you. But with just a few clicks, you’ll be back into your account in no time!
