Enrollment Procedures Grades Preschool through 12: Enroll students at the Groveport Madison Central Registration Office 5940 Clyde Moore Drive, Suite B Groveport, Ohio 43125 Phone: 614.491.8288 Fax: 614.836.1094 By Appointment Only When enrolling, please bring the following: The Completed Registration Packet (available on right hand side under Enrollment Forms) A copy of parent/guardian driver's license or State ID A copy of parent/guardian current lease or mortgage Any two (2) of the following: tax bill current utility bills insurance statement bank statement home closing purchase form Student's original birth certificate Current immunization record and physical form for Preschool and Kindergarten students Grade transcript (if applicable) Court documents- if applicable (custody papers or resident affidavit) Special Education file (MFE, IEP) Students who are not native US citizens are required to present shot records and TB test results  Kindergarten Enrollment and Eligibility: Children must be five (5) years of age by August 1, of the year in which registered.  Parents may enroll their child for Kindergarten beginning in March for the following year.  Call 614.491.8288 for further information.

Extended Day Program

The Extended Day Program is a before and after school child care program for children in grades K-5, who reside in the Groveport Madison Local School District.  The purpose is to provide affordable, quality child care, that instills a sense of pride for both school and community.  The program is located at Madison, Glendening and Sedalia Elementary.  Transportation is provided to and from school.
